Do you have a daily checklist for something or other? As part of your processes, you ask someone to complete a checklist, daily, every day, for the same thing they did the day before?[…]
You can’t judge how compliant, experienced or competent your people are on the amount of accidents, incidents or health issues you have or don’t have….

We are, at this moment in time, the sum of everything we have experienced and done. In 10 or 20 years time, we will be a different people… we will be the sum of what that person has experienced and done. That’s good news, because we can, with a bit of effort, future design ourselves, to be, someone a bit or a lot different.[…]
Unless you give a voice to your values, they are just meaningless words on some wall, webpage or lifeless policy document.

How do you know? What are you measuring? What does better look like? There’s the obvious; less harm done, measure… But that isn’t a measure of improvement, it’s a measure of the absence of harm…[…]
The assumption that you can make a behaviour happen by creating a form to be filled in, handing out said form and auditing its completion…
We are forever changing, we are become different people as we move through different environments.

Close calls are filled in to meet the quota of close calls needed to be filled in.
Close call programmes don’t directly improve safety.
If safety improves, it wasn’t the close call reporting that led to safety improvement…
